Which Page is called the perfect optimized page? If anyone tell you to optimize a page perfectly then how could you do? Which factors you should remember to make it perfect? Which on-page task you should implement to make your page perfect?

First check the meta tags, meta tags are present in the case or not. My second movement of the third and address labels to the labels in bold. Then look for keywords density. Checking robots.txt or sitemap.

There are so many things you need to optimize in page but in this the most important three things are.
1> meta title
2> meta description
3> meta keywords.
Optimizing this three things are more necessary.

As the old sarge is saying a page which follow Google webmaster guidelines, let discuss on this. There are many factors which are checked by Google and they start reading a page from top left to the right bottom. Here we have list of things which we can look at in a page.
1. Title - with one or two keywords only and should be written into descriptive form.
2. Description - two lines about the company help to get better CTR in SERPs but there is no relation with SERPs position.
3. Write Heading tag and for that use(H1 to H6) HTML tags.
4. Good content with proper keyword density, keyword prominence and keyword proximity.
5. Good use of anchor tags for internal linking.
6. Avoid use of images, flash and java scripts.

First off there no such thing as perfect, the best practices for on site optimization is to build great content people in your industry will want to read share etc.. but this is the easy part, the hard part is getting the right traffic to see it.

I do not think there is specific perfectly optimized page except google site
